Settle to Sleep’s intake form is designed to gather essential information to create the most effective sleep solution and program for the client and their baby. The information and answers provided by the client will not be disclosed with anyone outside of Settle to Sleep and will only be viewed and discussed within the organisation to ensure the best possible care.
